The Privacy Act of 1974 requires written permission before a congressional office may make certain inquiries to a federal agency on your behalf. Please download, complete, and return a signed copy of the appropriate form.

Please complete the Privacy Release Form and return to one of Pete’s state offices or email a completed and signed form to casework@ricketts.senate.gov. This form is used for most all cases unless you have an issue related to U.S. Citizenship and Immigration Services or the State Department.
